We used Amazon CreateSpace. It's an affordable printing/proofing/marketing tool suite.
We found it easy to use and intuitive, with good documentation. As an all-in-one solution for authors, we recommend it.
Step by step:

|We ordered five proofs: 6"x9", color cover, B&W interior, 250 pages|
Edited or re-worked files can be re-uploaded as many times as you wish, and new proofs ordered
